Is there a sample report available?
I have not seen a five year report yet.
I am thinking it should be based off of NFPA 25, and may be different for each system.
Maybe create a template and build off of it, for each system, showing compliance with 25 requirements, and what was done and found.
The 5-year internal inspection is not typically done alone. It is usually a part of the larger inspection, testing and maintenance of the system and a single report is usually developed for all of the work that is done. At the same time as the internal inspection of the pipe, there also needs to be an internal inspection of the check valves, and an internal inspection of the wet alarm valves. This work would usually be done at the same time as the annual external inspection of sprinklers, pipe and other components.
The National Fire Sprinkler Association sells forms that summarize all of the work that needs to be done and the frequency with which it needs to be done. You can contact them at www.nfsa.org. Be very careful if you are just going to write a report for the internal pipe inspection without any of the other inspection, testing and maintenance descriptions because building owners may not understand the limited scope of your work. If you are going to just write a report of the internal pipe inspection, it should include disclaimers that explain the scope of the work and that this does not constitute a full NFPA 25 test or inspection. Then, you just need to describe what you did and what you found. There is no specific format that is required for the report.
