Flexible Cords for industrial Machinery

It states that flexible cords may not be used in permanent installations as specified below:

(1) As a substitute for the fixed wiring of a structure
(2) Where run through holes in walls, structural ceilings, suspended
ceilings, dropped ceilings, or floors
(3) Where run through doorways, windows, or similar openings
(4) Where attached to building surfaces

 

 

Does that structure includes Industrial machinery?

If the Industrial machinery is not permanent, will the flexible cord be permitted?

 

Can a plug be a form of disconnecting means for industrial machinery? If so, what would be the criteria?
